Making an Appointment
Your first appointment will consist of a consultation explaining your diagnosis and treatment options. A complex medical history will be needed on each patient before a treatment plan can be created. When the treatment plan is complete, an appointment will be made for the treatment.
Please complete the Patient Registration Form needed for your appointment and bring the following information with you at the time of your consultation appointment:
- Your surgical referral slip and any X-rays if applicable. If your dentist or physician has taken X-rays, please request that they forward them to our office.
- A legible list of the medications you are currently taking including milligrams and how often each medication is taken. This is mandatory for all patients taking any type of medication including over-the-counter medications, vitamins or supplements. Surgical procedures cannot be done without knowledge of your medications prior to your procedure.
- If you have insurance, bring a copy of your dental insurance card or insurance billing information. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation appointment.
